Level 1 Participation: $1,400 per day (includes exhibitor table and luncheon speaker)
Host lunch for a day

Note: Only two lunch spots are available. Contact us early to secure your preferred day.
Level 2 Participation: $600 per day (includes exhibitor table and sign at refreshment table)

Level 3 Participation: $350 per day

- You will receive a list of course participants who give us permission to distribute their names

- Special recognition to your company at opening and closing remarks and in workshop material

You may mail or fax the form if paying by MasterCard or Visa. If you prefer to pay by check, make the check payable to: Pro Health Consulting and mail to the address on the form. Payment will be due unless cancellation notice is received two weeks prior to the start of the workshop.
